If you would like to be a vendor at this year's Hegewisch Fest, simply print and fill out one of the following forms, then mail or drop it off at:

Craft VendorsIf you are a craft vendor, use this form.

Not for Profits and Non-ProfitsIf you are part of a Not for profit or Non-profit organization, use this form.

Food VendorsIf you are a Food Vendor, you will need to fill out this Hegewisch Fest application form as well as the City of Chicago special single event food vendor application. But if you are a food truck, you will need to use the Food Truck Permit Application instead.
